Your first step is a conversation. When you call, our team takes the time to listen, understand what’s been going on, and answer your questions. There’s no pressure and no obligation — just a chance to talk through whether in-home therapy may be a good fit for you or your loved one.
A referral from a physician is required for all patients. This is a normal part of the process, and we help make it easy.
After our initial conversation, our team verifies your insurance coverage and Medicare benefits. If you already have a referral, we’ll review it with you. If not, we can coordinate directly with your doctor’s office to request the required prescription for therapy. Our goal is to handle these details for you so nothing feels overwhelming.
Once insurance is verified and the referral is in place, we schedule an evaluation at your home. A licensed therapist comes to you to assess strength, balance, mobility, safety, and daily activities. This visit helps us understand your goals and how therapy can best support independence at home.
Based on the evaluation, your therapist creates a personalized therapy plan focused on safety, strength, confidence, and everyday function. Therapy is delivered in your home and tailored to your specific needs, abilities, and goals.
You’ll work with the same trusted therapist every visit, allowing for continuity of care, steady progress, and a strong therapeutic relationship. As therapy continues, your therapist adjusts exercises and activities as you make progress, focusing on real-life movement that carries over into daily life. Families are kept informed, and care evolves as needs change.
